Mechanism of Action
Negative pressure interacts with the wound environment on different levels. It works on both the lesion’s macrostructure and microstructure and combines all these effects into one complicated operation that facilitates wound healing in all wound growth phases. At the macro level, NPWT removes secretions and fluids from the lesion, reduces periwound edema, increases local blood flow, and reduces ulcer size and the risk of wound contamination from external sources.1 At the micro level, the stimulation that the negative pressure exerts on the cell surfaces and on many of the cellular elements present in the lesion (in parallel with a change in nuclear transcription activity) induces positive changes in both the shape and function of the cells. NPWT has been shown to promote angiogenesis, fibrogenesis and the activity of macrophages and leukocytes. In virtually all clinical conditions related to wounds and wound management, acute and chronic, from heart surgery to diabetic feet and venous leg ulcers to pressure ulcers, a number of NPWT studies have been performed. Activity of the treatment has been confirmed for most of the conditions under which it has been tested. An interesting option with NPWT is that it can be incorporated into an antiseptic application. This occurs when antiseptics have entered the system to regularly flush the wound, or when they are absorbed into the material used to fill the lesion. This adds an antimicrobial effect to the indirect debridement of NPWT, as the newer antiseptics have a better efficacy and safety profile and have been tested successfully under a range of conditions.
Creating a Sub-atmospheric Pressure with Vacuum Source
The film has an opening to which it attaches a tube. The tube leads to a vacuum pump and a canister that collects liquids in it. It is possible to configure the vacuum pump mode to operate intermittently or to start and stop. The vacuum pump draws fluid and infection out ofthe wound. This will help bring the edges of the wound together. By promoting new tissue growth, it also allows the wound to heal. If required, antibiotics and saline solutions may be introduced into the wound.
The Provision of Oxygen to Wound Bed
Oxygen is the key essentials of all phases of wound healing. It acts as a nutrient for cell metabolism, phagocytes and other cells of the immune
system which converts oxygen into reactive oxygen species (ROS) that are then used for wound healing. Oxygen also promotes angiogenesis
through inducing vascular endothelial growth factor (VEGF) transcription and myofibroblast production and functions as a cofactor in collagen
synthesis. Lack of oxygen on the wound bed hinders the healing process.
The oxygen needs to go deep into the transdermal layer. Minus™ Negative Pressure Wound Therapy System delivers O2 to wound bed by low
frequency O2 delivery system. Thus, the wound bed is fed with more oxygen which the healing process needs.8
1. Maintaining Wound Temperature and Moisture
When the temperature drops, the healing stops. Furthermore, cooled tissues cause vasoconstriction and increase hemoglobin’s need for oxygen. As a result, there’s less oxygen available for the type of white blood cells called neutrophils to fight any potentialinfection. Once the wound tissues cool– such as when left open to air during a dressing change– the wound base can take up to4 hours to return to normal healing temperature. If a clinician changes a dressing TID, the wound may be outside of the optimal healing range 50% of the time. Minus™ Negative Pressure Wound Therapy System provides thermally-conditioned oxygen rich solution to wound bed at certain predetermined intervals. Thus, as temperature is kept at optimum healing level at 38 C, the temperature loss is eliminated. Moreover, the solution will not only help wash the wound bed at target temperature and dwell to penetrate dead corners of wound edges, but also help keep the moisture without macerating the tissue perimeters.
2. Wound Infection and Inflammation
Wound cannot be healed in presence of debris, necrotic tissue and infection. There should not be not only an effective debris and necrotic tissue removal, but also a solution for infection should be provided. Mechanical removal of infection on the wound surface is important to a point; however, in general, dead corners of wound cannot be addressed. Minus™ Negative Pressure Wound Therapy System also uses oxygen rich solution to help fight with infection on the surface level and dead corners with the aid of dwelling function. It allows user to set dwelling time depending on the solution they deliver to wound bed in case they do not want to use oxygenated solutions.
Indications
MINUS™ Negative Pressure Wound Therapy System is a non-pharmacological method for the patients who would benefit from a suctiondevice. It is intended to use to physically promote wound healing by means of removal of fluids, including irrigation, and body fluids wound exudates and infectious materials. The patients with the following wounds are indicated: Acute Wounds, Chronic Wounds Pressure Ulcers, Diabetic Foot Ulcers, Venous Leg ulcers, Traumatic wounds, Dehisced wounds, Partial thickness burns, Flaps, and grafts. No Studies are done on the use of Minus™ NPWT System on pregnant and pediatric patients. To use the device on such patients is at surgeons’ own disposal. Patient size and age should be considered when prescribing the device.
